Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the team.
• Carry out social evaluations, plan interventions, and provide crisis intervention when needed.
• Educate on cultural needs, advance directives, and care planning.
• Maintain clinical records for social work referrals.
• Provide information and referrals to community agencies.
• Serve as liaison between patients, families, and agencies.
• Participate in interdisciplinary care planning meetings.
• Assist with funeral, financial, legal, and health care decision planning.
