Infinity Hospice Care LLC is seeking a Master of Social Work to supervise Medical Social Workers under physician direction in our hospice program. The MSW will support patient/family psychosocial needs, participate in care coordination with the IDG/Team, and contribute to on-call coverage.

Qualifications

• Graduate of Medical Social Work degree as a Licensed Medical Social Worker.
• Master of Social Work degree required; LMSW licensure required.

Responsibilities

• Provide supervision for Medical Social Workers consistent with training and experience.
• Be readily available when social services are being provided; conduct documented conferences.
• Assess and complete Comprehensive Assessments within five days of hospice election.
• Assist with advance directives, discharge planning, and family counseling.
• Attend IDG/Team meetings and coordinate care with the IDG.
• Participate in on-call rotation and meet productivity standards.
• Communicate effectively with patients, families, and health professionals.
