Infinity Hospice Care LLC is seeking a Medical Social Worker to provide comprehensive psychosocial assessments and referrals in home-based settings across multiple states. The role involves evaluating client needs, coordinating with the care team, and supporting patients and families through care planning and transitions.

Qualified candidates will have an MSW with at least 1 year in a healthcare setting, strong communication skills, and an ability to navigate public and private resources.

Qualifications

• MSW skills defined as generally accepted standards of practice.
• Excellent interpersonal skills and ability to communicate effectively.
• Must read, write, and comprehend English.
• Maintain current MSW Certification in the state(s) of practice and CPR certification.

Responsibilities

• Perform psychosocial assessments, analysis, counseling and referrals.
• Assist in admission by evaluating psychosocial, financial, environmental and community needs.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Accept assignments consistent with education and competence.
• Provide clear communication to clients, families and team members; participate in meetings.
• Maintain ongoing professional development and continuing education requirements.
