Eternal Faith Hospice in Dalton, GA seeks a qualified Master of Social Work to supervise Medical Social Workers and coordinate care with the interdisciplinary team. The MSW will provide patient- and family-centered psychosocial support, coordinate with care teams, and participate in on-call rotations.

The role requires licensure as an MSW, a strong understanding of hospice standards, and the ability to manage assessments, counseling, and documentation with compassion and professionalism.

Competenze

• Graduate degree in Social Work (MSW) and licensure as an MSW.
• Current state license to practice medical social work.
• Experience with hospice/palliative care preferred.

Mansioni

• Provide supervision for Medical Social Workers in line with training and patient needs.
• Be readily available when social services are provided and participate in on-call rotations.
• Lead conferences with Medical Social Workers on patient care and determine meeting frequency.
• Complete comprehensive psychosocial assessments within five days of hospice election.
• Assist with advance directives, discharge planning, and family counseling.
• Attend IDG/Team meetings to coordinate patient care and resources.
• Ensure privacy, infection control, and timely paperwork submission.
• Maintain engagement with hospice education and QAPI participation.
• Communicate effectively with patients, families, and health professionals.
