Job summary

Affinity Hospice Management LLC is seeking an LPN to provide home-based hospice care. The role works with an RN to assess patient and family needs, administer medications, educate families, and coordinate care in the home setting.

The ideal candidate is an LPN in good standing with relevant experience and strong communication skills. You must be able to lift patients and drive as required to fulfill duties.

Qualifications

• Graduate of an accredited nursing program.
• Licensed Practical Nurse (LPN) in good standing.
• Minimum 2 years nursing experience in last 3 years, or 1 year with hospice/public health/home care.
• Background checks and references as required.
• Strong observation, communication, problem-solving, and math skills.
• Must be able to lift and transfer patients; driving required.

Responsibilities

• Complete routine nursing assessments to determine hospice needs.
• Provide nursing care using the nursing process and reassess patient status.
• Administer medications and treatments per physician plan.
• Counsel patients and families on nursing and self-care.
• Collaborate with RN Case Manager and Clinical Supervisor.
• Maintain accurate clinical notes and report changes promptly.
• Participate in on-call duties and ensure equipment and services are available.
