Job summary

Apploi is seeking a Licensed Practical Nurse (LPN) to provide hospice care in home and community settings. The LPN will work closely with RNs to assess and meet the needs of patients and families, administering medications, teaching self-care, and coordinating care across the care team.

The role requires an active LPN license, strong communication skills, and the ability to transport to patient homes. Experience in home health or hospice nursing is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• LPN with a license in good standing to practice.
• Minimum two years nursing experience in last three years, or one year with HIV/public health/home care/hospice exposure.
• Offer contingent on references and criminal background checks.

Responsibilities

• Perform routine nursing assessments to determine hospice needs.
• Provide nursing care using the nursing process and plan of care.
• Re-evaluate patient and family needs and adjust care.
• Administer medications and treatments per physician's orders.
• Counsel patient/family on nursing needs and self-care.
• Provide health instructions and promote patient comfort.
• Assist with activities of daily living and mobility.
• Collaborate with RN Case Manager and Clinical Supervisor.
• Attend interdisciplinary meetings.
