Qualifications

Responsibilities

• Complete routine nursing assessments to determine hospice needs.
• Provide nursing care per the nursing process.
• Reassess patient status and adjust care plans as needed.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing needs and care options.
• Deliver health education to patients and caregivers.
• Assist with activities of daily living and promote independence.
• Collaborate with RN Case Manager and Clinical Supervisor to coordinate care.
