Qualifications

• Deliver after-hours care, including in-person visits as needed.
• Respond to calls from patients and caregivers with symptom assessment and education.

• Document encounters accurately in the EMR with clear handoffs.
• Travel to patient homes and community settings as required.
• Maintain compliance with hospice policies and regulatory requirements.
