Job summary

Qualifications

• Graduate of an accredited nursing program with current state licensure and CPR.
• 1–2 years recent acute care and home care experience preferred.
• Licensed driver with insured vehicle required.

Responsibilities

• Performs comprehensive patient/family assessments to determine hospice needs.
• Provides nursing care using the nursing process and plans of care.
• Evaluates patient status and initiates or revises care plans.
• Reassesses patient and family needs regularly and updates plans.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and care needs.
• Communicates with physicians and hospice team regarding changes in patient status.
