Advocate Health is seeking a Part-time Hospice Registered Nurse to support the Supportive Care Team in Illinois. The role involves weekend shifts (Saturday and Sunday, 8am-8pm), after-hours triage, and in-home patient visits to manage end-of-life symptoms with empathy and clinical precision.

Required are an active RN license, BLS, and at least one year in hospice or related settings. Travel to patient homes is expected as part of the job, with comprehensive EMR documentation and collaboration

Competenze

Experience in hospice or palliative care preferred.
Ability to assess and triage complex symptoms for hospice patients.
Strong communication with patients, families, and care team.

Mansioni

Provide after-hours nursing assessment, triage, and interventions for hospice patients.
Respond to calls, educate caregivers, and determine appropriate interventions.
Conduct in-person visits for acute symptom management and patient comfort.
Document encounters accurately in the electronic medical record.
Collaborate with physicians and interdisciplinary team to coordinate care.
