Job summary

Advocate Health Care seeks a PRN Hospice Liaison for the North Chicago market. The role promotes hospice services at partnered facilities, requires a valid driver’s license, reliable transportation, and flexibility across a Monday–Friday schedule with 8–16 hours weekly.

RN background and hospice experience are preferred. The position covers a 10422 IL Home Health Divisional patient access area, with collaborative duties across physicians, nurses, social workers, and care managers to ensure

Qualifications

• Valid RN license and current state license.
• Prior hospice or home health discharge planning experience preferred.
• Strong communication and collaboration with care teams.

Responsibilities

• Educate patients and families about home hospice services and plan of care.
• Coordinate referrals and admissions to home health and hospice services.
• Collaborate with physicians, social workers, and care managers on complex cases.
• Travel to PAN/SNF facilities and patient homes as required.
