Job summary

Affinity Health Management is seeking a Hospice Chaplain to deliver spiritual care to patients and families, coordinating with the hospice team. You will assess spiritual needs, plan care, and provide direct support while liaising with community chaplains and counselors.

The role involves documentation, participation in IDG meetings, and education for the interdisciplinary group. Experience in hospice care and CPE are highly valued, with flexibility to engage across faiths and cultures.

Qualifications

• Bachelor's degree in religious studies or related subject and a master's degree in theology, divinity or pastoral studies is required.
• Certification is preferred; ordination/endorsement may be required depending on affiliation; MA in Theology or related subject and four CPE units commonly expected.
• Ongoing commitment toward development through education and obtaining CPE.
• Experience in hospice care preferred.
• Comfort in an interfaith setting and ability to work with diverse beliefs and cultures.
• Ability to network with community clergy and congregations.
• Must have CPR certification and a valid driver's license with insured vehicle.
• Background checks required after offer; reports to Dept. of Health as mandated.

Responsibilities

• Assures spiritual assessment of patients and families/caregivers in the hospice program and timely services.
• Develops plan of care by participating in comprehensive assessment of spiritual needs.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and program development.
• Documents direct services and communication with community chaplains and counselors.
• Attends IDG meetings and coordinates with the interdisciplinary group on spiritual care.
• Provides consultation, education, and support to the interdisciplinary group.
• Recruits community chaplains and volunteers to meet patient/family needs.
• Provides funeral/memorial services as requested and plans periodic memorials.
