Job summary

Affinity Hospice Care, Inc. is seeking a Hospice Chaplain to provide spiritual care to patients and families, coordinating with a multidisciplinary team. The role includes assessments, care planning, documentation, and participation in care conferences to ensure compassionate end-of-life support.

The chaplain will network with community clergy, participate in quality initiatives, and help organize memorial services as requested, while supporting volunteers and staff.

Qualifications

• Graduate of accredited seminary or school of theology (BD, Mdiv or equivalent theological degree).
• Experience with death and dying, individuals/family/caregiver.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Assess spiritual needs and develop care plans with the team.
• Document services and coordinate with community chaplains.
• Participate in care conferences and interdisciplinary meetings.
• Recruit community chaplains and volunteers to meet needs.
• Support quality improvement and memorial services as needed.
• Supervise spiritual care volunteers when assigned.
