Job summary

Affinity Health Management is seeking a Hospice Chaplain to provide spiritual care to patients and families, coordinating with the interdisciplinary team to meet identified spiritual needs. Duties include assessment, plan of care development, direct care, and collaboration with community chaplains and volunteers.

On-call responsibilities and ongoing education are part of the role. Candidate must hold a relevant bachelor’s and a master’s in theology or pastoral studies, with CPE experience

Qualifications

• Bachelor's degree in religious studies or related subject.
• Master's degree in theology, divinity or pastoral studies; 72 semester hours.
• CPE internship or one unit (400 hours) preferred.
• Ordination/commissioning and endorsement letter preferred for chaplaincy.
• Strong communication, networking and interfaith awareness.

Responsibilities

• Provide direct spiritual care to patients and families.
• Assess spiritual needs and develop plans of care with the interdisciplinary team.
• Coordinate with community chaplains and spiritual counselors.
• Document services and maintain records for QA and compliance.
• Attend care conferences and support the interdisciplinary group.
• Recruit and educate community chaplains and volunteers.
• Provide memorial services and ongoing spiritual support as requested.
• Participate in on-call duties and ongoing professional development.
• Support continuity of care across the hospice team and external partners.
