Job summary

Chapters Health System in Florida is seeking an Admission RN to assess patients at their residence or in hospitals and initiate care within our hospice program. You will ensure a seamless transition to Care Navigation and serve as a company ambassador.

Requirements include active RN license in the state, at least 1 year of nursing experience (hospice or hospital preferred), EMR/EHR experience, and BLS certification. The role offers a day shift with rotating weekends and a sign-on bonus.

Qualifications

• Active RN license in the state where working.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• Bi-lingual in Spanish preferred; English communication required.

Responsibilities

• Represent the Company professionally in care and services.
• Comply with state, federal, local regulations and Company policies.
• Observe health, safety and security practices; protect patient confidentiality.
• Coordinate care transitions with Care Navigation and facilities.
• Initiate Plan of Care and order physician-initiated interventions when needed.
• Maintain timely documentation per Company policy.
