Job summary

Chapters Health System is seeking an RN for Hospice House to assess patient and family needs, coordinate care, and provide compassionate palliative support. The role requires active RN licensure, hospice/home health experience, EMR proficiency, strong communication, and flexibility to travel as needed.

The Hospice House RN will work within an Interdisciplinary Group to develop individualized care plans, educate families, and monitor patient status.

Qualifications

• Active RN license in the state of practice.
• Hospice/home health experience preferred.
• Experience with EMR/EHR systems.
• BLS for healthcare professionals required.
• Ability to travel off-site as needed.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient, family and caregivers about disease, palliative care, safety and decline.
• Reports changes in patient condition to IDG or other health professionals.
• Participates with IDG to evaluate hospice referrals/admissions for level of care.
• Attends patient deaths; prepares body for transport; oversees disposal of medications.
• Provides bereavement interventions to family with psychosocial staff.
• Presents concise reports to IDG; encourages input from all disciplines.
• Communicates with physicians, staff, patients and families; promotes collaboration.
• Supervises care provided by LPNs and contract nurses; performs other duties.
