Job summary

Affinity Health Management in Beachwood, OH, seeks a Registered Nurse with hospice and home health experience to plan, organize and direct patient care within the community. The RN will collaborate with the inter-disciplinary team and family caregivers to meet patient goals.

Responsibilities include comprehensive assessments, care planning, medication administration, education, and ongoing communication with physicians and team members to ensure continuity of care within home settings.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current RN license in Ohio.
• Minimum 1 year of professional nursing in the last 3 years; or a BSN with RN license; 2 years in public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient and family.
• Provide professional nursing care per the nursing process.
• Develop and revise the plan of care with patient and family input.
• Initiate and revise the plan of care as patient needs change.
• Administer medications and treatments prescribed by the physician.
• Counsel patient and family and provide health instructions.
• Coordinate care as Case Manager when assigned.
• Maintain accurate clinical notes and communicate changes to physicians.
