Job summary

Affinity Health Management seeks a qualified Registered Nurse to plan, organize and direct hospice care in home and community settings in Ohio. You will use nursing processes to assess patients and families, develop care plans, and coordinate with the interdisciplinary team to meet patient goals.

Responsibilities include comprehensive assessments, implementing care plans, administering medications, educating families, and contributing to on-call coverage.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum of one year of nursing experience within last three years, or have a BSN with RN license and two years' experience (public health/home care/hospice preferred).
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Initiate plan of care and revise as patient status changes.
• Administer medications and treatments as prescribed.
• Educate the patient and family on self-care and care needs.
• Coordinate with physician and interdisciplinary team for care planning.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise ancillary personnel as required.
• Engage in quality improvement activities and ongoing professional development.
