Qualifications

• Graduate of an Associate’s or diploma nursing program.
• One year nursing experience OR one year nursing experience in a specialty.
• CT license to practice nursing.
• Preferred: NLN approved BSN program graduate.
• Preferred: medical/surgery or home care experience.

Responsibilities

• Provide skilled nursing care to hospice clients in homes or community health settings.
• Conduct a complete nursing assessment based on referral data and interview.
• Develops a plan of care with client and interdisciplinary team.
• Facilitates bereavement follow up and adjusts plan as needed.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Coordinate care with physicians and other providers.
