Job summary

Hartford HealthCare at Home seeks a Hospice Registered Nurse to provide skilled nursing care to hospice clients in their homes or community settings in Mansfield region. The role emphasizes high-quality, outcome-based care with a patient-and-family-centered approach.

Requires CT license and at least 1 year of nursing experience; BSN preferred. The work involves collaborating with an interdisciplinary team and supporting bereavement follow-up and discharge planning.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: Education: NLN-approved BSN program.
• Preferred: Experience in medical/surgery or home care.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts a complete nursing assessment and documents plan of care.
• Develops a plan of care with client and all disciplines; facilitates discharge planning and bereavement follow-up.
• Demonstrates understanding of payer information to maximize care planning and uses Hospice levels of care appropriately.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Teaches patients and caregivers about signs, symptoms of death and home care.
• Coordinates all client care services and consults with physician as needed.
