Hartford HealthCare at Home is seeking a Hospice Registered Nurse to deliver high-quality, patient-centered care in patients' homes within Mansfield region. You will assess needs, develop care plans, coordinate services, and support families facing end-of-life challenges.

Join Connecticut’s leading home care provider and contribute to compassionate, outcome-based care. Must hold CT RN licensure and have a relevant nursing education; experience in hospice/home care is preferred.

Qualifications

• Graduate of an Associate’s or diploma program in nursing.
• CT RN licensure required.
• Hospice experience preferred.

Responsabilités

• Provide skilled nursing care to hospice clients in home settings.
• Review referrals and contact clients and families promptly.
• Perform comprehensive nursing assessments and document findings.
• Develop and modify care plans with the patient and care team.
• Coordinate with physicians and interdisciplinary team members.
• Educate patients and families on care plans and symptoms.
