Job summary

Hartford HealthCare at Home in Mansfield region seeks a Hospice Registered Nurse to provide skilled nursing care to patients in their homes, focusing on comfort and end-of-life support. You will assess needs, develop and document plans of care, and collaborate with families and the interdisciplinary team.

You will educate families, coordinate services, and ensure timely documentation while delivering compassionate, patient- and family-centered care within Connecticut communities.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: NLN-approved BSN program.
• Experience: Medical/surgery or home care experience.

Responsibilities

• Provides skilled nursing care to hospice clients in their homes or community settings.
• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and plans of care with the client and disciplines.
• Facilitates bereavement follow-up and coordinates care with providers.
• Directs LPNs and Home Care Aides in carrying out the care plan.
