Job summary

St. John’s United Home Health & Hospice seeks a Medical Social Worker (MSW) to deliver psychosocial services in home health and hospice settings. The MSW will contribute to a bereavement program and collaborate with a multidisciplinary team to support patients and families.

The role emphasizes discharge planning, clinical documentation, and adherence to CMS/Medicare regulatory requirements for home health and hospice, with travel to patient homes as needed.

Qualifications

• Assess the psychosocial status of patients and families.
• Carries out social evaluations and plans interventions or support services to patients and family/caregivers.
• Assists with maintaining a bereavement program.
• Maintains clinical records on all patients served.
• Participates in discharge planning as appropriate.

Responsibilities

• Provide living opportunities within nurturing environments of hope, dignity and love.
• Follows safety rules and attends mandatory safety in-services.
• Engages in customer service aligned with the Eden Alternative philosophy.
• Daily communication with potential patients, families, and nursing home staff.
• Adheres to policy on TB testing and annual influenza vaccinations.
