Job summary

John Paul VNA Hospice - a LiveWell Partners Network Agency is seeking a Hospice Medical Social Worker (MSW) to provide psychosocial support to patients and families throughout the hospice journey. This role involves assessing needs, planning care with the interdisciplinary team, and advocating for patients in end-of-life care and bereavement processes.

Qualifications

• Active Michigan social work licensure required.
• MSW degree required.
• 1 year of licensed social work experience in healthcare setting required.

Responsibilities

• Conduct comprehensive psychosocial assessments of hospice patients and families, identifying emotional, social, financial, environmental, and practical needs.
• Develop and implement individualized social work interventions with counseling, crisis intervention, and end-of-life planning support.
• Educate patients and families about hospice philosophy, services, and advance care planning.
