Job summary

Duke HomeCare & Hospice in Durham, NC seeks a Registered Nurse for Hospice Home Care (PRN). You will provide professional nursing care in home or hospice settings, guided by NC nursing practice acts and agency policies.

Candidates should have NC RN licensure, current CPR, and at least two years of med-surgical or home health/hospice experience. Must travel to patients' residences and hold a valid driver's license.

Qualifications

• Associate Degree or Diploma, Nursing required.
• Must have RN licensure in the state of North Carolina (NC).
• Two years of med-surgical, critical care or home health/hospice/infusion experience.

Responsibilities

• Provide professional nursing care in home or alternate site setting according to nursing practice and agency policies.
• Perform assessments, plan and document care.
• Coordinate with interdisciplinary team and physicians to ensure quality patient care.
• Develop and implement discharge plans and educate patients and families.
