Job summary

Duke HomeCare & Hospice in Durham, NC is seeking a PRN Registered Nurse to deliver professional nursing care in home-based settings, following the North Carolina Board of Nursing guidelines.

The role emphasizes plan development, patient education, and interdisciplinary collaboration to ensure quality patient outcomes. Requires NC RN licensure, BLS, and prior hospice/home health experience where applicable.

Qualifications

• Associate Degree or Diploma in Nursing required.
• Current North Carolina RN licensure or compact licensure.
• BLS Certification required.
• Two years of med-surgical, critical care or home health/hospice/infusion experience.
• Prior hospice experience preferred.

Responsibilities

• Plan, provide, supervise and document professional nursing care using the nursing process.
• Collaborate with the healthcare team to evaluate care and outcomes.
• Assess educational needs of patients and families regarding health changes and care plans.
• Develop and implement discharge plans at initiation of services.
• Maintain up-to-date knowledge of guidelines and DHCH policies; participate in team meetings.
