Duke HomeCare & Hospice in Durham, NC is seeking an experienced Registered Nurse for PRN on-call hospice care in patient homes and our facility. The On-Site role focuses on end-of-life support, symptom management, and coordinating with physicians and care teams to ensure compassionate, high-quality care.

The RN carries an Associate Degree or Diploma in Nursing with NC or compact state license, CPR, and at least two years of med-surgical, critical care, or home health/hospice experience.

Qualifications

• Active North Carolina RN license or compact state.
• CPR certification (American Heart Association).
• Minimum two years of med-surgical, critical care, or home health/hospice experience.
• Prior hospice experience required.
• Valid driver’s license and auto insurance.

Responsibilities

• Provide professional hospice nursing care in home or alternate settings using the nursing process and regulatory standards.
• Assess patient conditions, manage symptoms, and communicate significant changes to physicians and care teams.
• Educate patients and families on disease progression, medications, and end-of-life care planning.
• Collaborate with interdisciplinary teams to coordinate care, support discharge planning, and ensure quality outcomes.
• Maintain accurate documentation, ensure compliance, and support performance improvement initiatives.
