Job summary

Duke University Health System's Duke HomeCare & Hospice in Durham, NC is seeking a compassionate RN to provide hospice care in patients' homes and on-call visits. This PRN role focuses on delivering high-quality, patient-centered end-of-life care across central NC.

The position requires an RN with an ADN/ diploma, NC license, CPR, and experience in home health or hospice; strong assessment and communication skills are essential, along with ability to travel and work independently.

Qualifications

• Associate Degree or Diploma in Nursing required; BSN encouraged after two years.
• Current RN license in North Carolina or compact state.
• CPR certification (American Heart Association).
• Minimum two years of med-surgical, critical care, or home health/hospice experience required.
• Prior hospice experience required.
• Valid driver’s license and current automobile insurance.

Responsibilities

• Provide professional hospice nursing care in home or alternate settings using the nursing process and regulatory standards.
• Assess patient conditions, manage symptoms, and communicate significant changes to physicians and care teams.
• Educate patients and families on disease progression, medications, and end-of-life care planning.
• Collaborate with interdisciplinary teams to coordinate care, support discharge planning, and ensure quality outcomes.
• Maintain accurate documentation, ensure compliance, and support performance improvement initiatives.
