Job summary

Duke HomeCare & Hospice in Durham, NC is seeking a dedicated registered nurse to provide professional nursing care in patients' homes or our inpatient hospice facility. You will collaborate with physicians and care teams to deliver high-quality, compassionate care tailored to each patient and family.

The role emphasizes patient education, discharge planning, and adherence to nursing standards. BLS and NC RN licensure are required; BSN pursuit is encouraged after two years of service.

Qualifications

• Associate Degree or Diploma, Nursing required.
• RNs encouraged to enroll in a DUHS approved BSN program after two years of service.

Responsibilities

• Plan, provide, supervise and document professional nursing care utilizing the nursing process.
• Assess educational needs of patients/families concerning health changes and plan of care.
• Develop and implement an effective discharge plan for patients starting at service.
• Coordinate with the healthcare team to ensure safe, cost-effective patient care.
• Maintain up-to-date knowledge of standards and document care per policy.
