Qualifications

• Associate Degree or Diploma in Nursing.
• Active North Carolina RN licensure.
• BLS Certification.
• Two years of med-surgical, critical care or home health/hospice/infusion; hospice experience preferred.

Responsibilities

• Plan, provide and document professional nursing care per policy and physician orders.
• Use nursing judgment to individualize the care plan.
• Assess educational needs of patients and families and plan outcomes.
• Develop discharge planning to begin at initiation of services.
• Educate patients/families and evaluate teaching outcomes.
• Model professional nursing conduct and ethics.
• Participate in weekly interdisciplinary team meetings and case conferences.
• Maintain knowledge of standards and participate in in-services.
• Document care in DHCH systems.
• Coordinate with billing staff to ensure accurate billing.
