Job summary

Duke HomeCare & Hospice is seeking a Registered Nurse for PRN on-call evenings to deliver compassionate, patient-centered hospice care in patients' homes across central North Carolina. The role emphasizes symptom management, education, and coordination with physicians and interdisciplinary teams.

The RN will perform assessments, provide end-of-life care planning, and ensure meticulous documentation while supporting on-call coverage and travel to patient sites.

Qualifications

• Associate Degree or Diploma in Nursing is required.
• BSN encouraged after two years.
• Current RN license in North Carolina or a compact state.
• CPR certification (American Heart Association).
• Minimum two years of med-surgical, critical care, or home health/hospice experience.
• Prior hospice experience required.

Responsibilities

• Provide professional hospice nursing care in home or alternate settings using the nursing process and regulatory standards.
• Assess patient conditions, manage symptoms, and communicate significant changes to physicians and care teams.
• Educate patients and families on disease progression, medications, and end-of-life care planning.
• Collaborate with interdisciplinary teams to coordinate care, support discharge planning, and ensure quality outcomes.
• Maintain accurate documentation, ensure compliance, and support performance improvement initiatives.
