Job summary

UnitedHealth Group's Patient Care Hospice seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in home settings across New Jersey. You will collaborate with the care team to ensure physical, psychosocial, emotional, and spiritual well-being and monitor care quality.

Requires current NJ RN license, dependable transportation, and home-care experience. Travel within Atlantic, Cumberland, Gloucester, Cape May, Ocean, and Camden counties within a 1-hour radius of Egg

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license with access to dependable transportation.
• 1+ years of clinical home-care experience preferred.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral.
• Identifies needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with patient/family, physician and IDG daily.
• Instructs and supervises in self-care when appropriate.
• Maintains accurate clinical notes on patient condition.
