Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for Patient Care Hospice, part of LHC Group, to deliver full nursing care for hospice patients in-home across New Jersey.

Responsibilities include initial evaluation within 48 hours of referral, developing and implementing the Plan of Care, and coordinating with the patient, family, physician, and IDG. Travel within Atlantic, Cumberland, Gloucester, Cape May, Ocean, and Camden counties within a 1-hour radius is required.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver''s license with access to a dependable vehicle, or public transportation

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and reassess as needed, at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, attending physician and IDG daily.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes regarding the patient''s condition
