Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to deliver hospice care in patients' homes across New Jersey. The role requires an active RN license, home-care experience, and the ability to travel within a 1-hour radius from Egg Harbor, NJ.

The position involves evaluating patient needs, developing and implementing care plans, and coordinating with the healthcare team to promote holistic well-being. On-call hours and a PRN scheduling model are included.

Qualifications

• Active RN license in the state of practice.
• 1+ years of clinical experience preferred.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Perform initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess patient/family needs and adjust care plans as needed (at least every 15 days).
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG members to provide care daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
