Job summary

Optum's Patient Care Hospice, part of LHC Group, is hiring a Registered Nurse Case Manager in New Jersey to deliver hospice care under the Plan of Care and collaborate with the IDG to ensure quality outcomes. The role emphasizes compassionate, holistic care for patients and families facing end-of-life decisions.

Requires current NJ RN licensure, a valid driver's license, and reliable transportation. Travel across multiple NJ counties with on-call evenings and weekends; familiar with hospice

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify needs and re-assess at least every 15 days; initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG for daily care.
• Instruct and supervise patient/family in self-care techniques; maintain accurate notes.
