Job summary

UnitedHealth Group seeks a Registered Nurse Case Manager to deliver hospice care in patients’ homes across New Jersey. You will evaluate needs, develop a Plan of Care with the team, and implement evidence-based strategies to support physical, psychosocial, and spiritual well-being.

The role requires an active NJ RN license and travel within a 1-hour radius from Egg Harbor, with 48-hour evaluations and ongoing family collaboration. CPR certification preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice (New Jersey).
• Current driver’s license, reliable vehicle or access to transportation.
• Home-care experience preferred.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs and reassess at regular intervals.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, physician, and IDG members for daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes about the patient’s condition.
• Adapt to home situations regardless of client demographics.
• Provide performance feedback and opportunities for growth.
