Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to provide in-home hospice care within New Jersey. You will evaluate eligibility within 48 hours of referral and coordinate the care plan with the patient, family, and IDG teams.

The role requires NJRN licensure, a valid driver's license, reliable transportation, and home-care experience. CPR certification or ability to obtain within 90 days is preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice (New Jersey).
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• Home-care experience preferred for this position and CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess physical, psychosocial, and environmental needs; initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses; collaborate with the IDG daily.
