Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for in-home hospice care in New Jersey. The role involves evaluating patients, developing plans of care, and coordinating with families and providers to ensure quality end-of-life care.

Travel within Atlantic, Cumberland, Gloucester, Cape May, Ocean, and Camden counties is required, with an emphasis on a 1-hour service radius from Egg Harbor. CPR certification is preferred, and RN licensure must be current and unrestricted.

Qualifications

• Home-care experience is required.
• Must be an RN with current unrestricted licensure in NJ.
• On-call availability evenings and weekends is required/intended.

Responsibilities

• Assume full nursing responsibility for delivery of the Plan of Care.
• Initial nursing evaluation within 48 hours of referral.
• Develop and adjust Plan of Care with the care team.
• Document patient status and interventions.
• Collaborate with patient/family and physicians daily.
• Instruct patient/family in self-care techniques when appropriate.
