Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for in-home hospice visits in New Jersey. You will evaluate patients, develop and implement care plans, and collaborate with families and physicians to promote quality of life for hospice patients.

The role requires NJ RN licensure, a valid driver’s license, and willingness to travel within a 1-hour radius from Egg Harbor, NJ. At least 1 year of clinical experience and Home-Care background are preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience (preferred).
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician, and IDG members daily.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes.
• Function in diverse home situations regardless of client background.
• Be rewarded for performance and able to develop other roles.
