Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients with in-home visits across designated NJ counties. You will evaluate patient needs, develop and implement care plans, and coordinate with the care team to ensure compassionate, comprehensive end-of-life care.

Responsibilities include initial evaluation within 48 hours, ongoing reassessment, and patient/family education. Travel within a 1-hour radius from Egg Harbor, NJ is required.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• Valid driver's license and access to a dependable vehicle, or reliable public transportation.
• Home-care experience preferred.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care and documents interventions and responses.
• Collaborates with patient/family, attending physician and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Deliver care irrespective of age, race, creed, color, sex, disability, or financial condition.
