Job summary

Anchor Health is seeking a Hospice Clinical Liaison to expand census and act as a positive ambassador for the hospice program in Marin County. You will develop a structured outreach program to identify new referral sources and maintain relationships with physicians and partner facilities.

The role emphasizes independence, strong communication, and community engagement. The ideal candidate holds LVN/RN/MSW credentials with 1–2 years in healthcare marketing or sales and hospice experience

Qualifications

Must hold LVN, RN or MSW credentials.
1–2 years in healthcare marketing/sales.
Hospice experience preferred.
Familiarity with referral partners (hospitals, physicians, facilities).
Excellent communication and ability to work independently.

Responsibilities

Grow census and reflect positively on the hospice program.
Develop and implement contact and outreach with referral sources.
Support referral partners, patients, families and the team through the referral-to-admission process.
Build lasting professional relationships with physicians and hospitals.
Plan marketing events, educate stakeholders about hospice and palliative care.
Attend weekly meetings and report on opportunities for growth.
Meet quota and expectations set by the Director of Business Development.
