Job summary

Anchor Health in Marin County is seeking a Hospice Clinical Liaison to grow the census and be a positive reflection of our hospice program. You will develop and implement a structured outreach program to identify new referral sources and maintain ongoing relationships with physicians, hospitals, and SNFs.

The role requires LVN licensure, 1-2 years of healthcare marketing/sales experience, and strong communication skills. A valid driver's license is also required.

Qualifications

• LVN certification and license required.
• 1-2 years of marketing/sales in healthcare preferred.
• Two years hospice experience strongly preferred.
• Familiarity with referral partners (physicians, B&C, ALF, RCFE & SNF).
• Excellent communication, independent work capability, interpersonal skills.

Responsibilities

• Grow census and reflect positively on the hospice program in the community.
• Develop and implement a regular outreach program to identify new referral sources.
• Maintain ongoing relationships with hospice partners, physicians, and referral sources.
• Support referral partners, patients, families, and team through the referral to admission process.
• Build lasting professional relationships with healthcare providers (physicians, hospitals, SNFs).
• Plan and organize marketing events, luncheons, and informationals.
• Educate stakeholders about hospice and palliative care.
