Job summary

Anchor Health is seeking a Hospice Clinical Liaison to advance census and serve as a positive ambassador within Riverside/San Bernardino County communities. The role develops structured outreach to cultivate new referral sources and maintains strong relationships with physicians, hospitals, SNFs, and other partners.

The ideal candidate is proactive, solutions-oriented, and excels at follow-through and collaboration across the referral-to-admission process.

Qualifications

• LVN/RN/MSW required.
• 1–2 years healthcare marketing/sales experience preferred.
• Minimum of 2 years hospice experience highly preferred.
• Familiarity with local referral partners (hospitals, physicians, B&C, ALF, RCFE).
• Strong communication skills and ability to work independently.

Responsibilities

• Build relationships with physicians, hospitals, SNFs, discharge planners, and other referral sources.
• Plan and implement marketing plans to promote census growth and referrals.
• Track referral trends and identify marketing opportunities.
• Coordinate with clinical team to align with Plan of Care and Medicare guidelines.
• Process workflow in EMR system and handle communications.
• Attend weekly meetings and report on successes and opportunities.
• Meet quotas set by Director of Business Development.
