Job summary

Anchor Health is seeking a Hospice Clinical Liaison to grow census and serve as a positive ambassador for our hospice program in Marin County. The role emphasizes relationship-building with physicians, hospitals, and referral sources, with a structured outreach plan and quota-driven growth.

Qualifications

1-2 years marketing/sales experience in healthcare
Two years hospice experience preferred
Familiarity with referral partners (physicians, B&C, ALF, RCFE & SNF)
Valid driver’s license, good driving record and current auto insurance
Excellent written and verbal communication, independent work capability

Responsibilities

Grow census and represent the hospice positively in the community
Develop and implement outreach to identify referrals and maintain relationships with partners
Support referral process from referral to admission
Meet quarterly quota set by leadership
Build relationships with physicians, hospitals, SNFs and discharge planners
Educate stakeholders about hospice and palliative care
Attend meetings to discuss referrals, competition, and growth opportunities
