Qualifications

• 2 years of sales experience in a clinical care setting (hospice preferred).
• Heart to serve patients and families; compassionate care focus.
• Reliable transportation and ability to work in a field-based role.

Responsibilities

• Serve as the primary liaison between the hospice agency and local healthcare providers.
• Generate hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams to ensure seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
• Maintain accurate referral documentation and communication with care teams.
