Job summary

Anchor Health is seeking a Hospice Community Liaison in California to grow census and serve as a positive ambassador for the hospice program. You will develop structured outreach, build lasting relationships with referral sources, physicians, and partner organizations, and work collaboratively through the referral-to-admission process.

The ideal candidate will have 1-2 years in healthcare marketing/sales, hospice experience preferred, and strong communication skills.

Qualifications

• 1-2 years of experience in marketing/sales in healthcare field.
• Two years’ experience in hospice preferred.
• Familiarity with referral partners in the area including physicians, B&C, ALF, RCFE & SNF.
• Must have great communication skills and ability to work independently.

Responsibilities

• Build relationships with physicians, hospitals, SNFs, discharge planners and related partners.
• Develop and implement strategic marketing plans to promote census growth and referrals.
• Track referral trends and identify key marketing opportunities.
• Plan and organize marketing events and host luncheons.
• Educate stakeholders about hospice and palliative care.
• Oversee initial patient consent and coordination with Clinical team.
• Attend weekly meetings and articulate opportunities for growth.
