Qualifications

• LVN, RN, or MSW required.
• 1-2 years healthcare marketing/sales experience preferred.
• Minimum of 2 years hospice experience highly preferred.
• Strong familiarity with local referral partners including Hospitals, Physicians, Board & Care (B&C), Assisted Living Facilities (ALF), Residential Care Facilities for the Elderly (RCFE), and Skilled Nursing Facilities (SNF).
• Excellent written and verbal communication skills.
• Self-directed with the ability to work independently.
• Determined, collaborative, and strong work ethic.
• Compassionate interpersonal approach.
• Valid driver’s license, clean driving record, and current auto insurance required.

Responsibilities

• Build and maintain relationships with physicians, hospitals, SNFs and other partners.
• Plan and coordinate marketing events, educational sessions, and community luncheons.
• Educate stakeholders on hospice and palliative care services.
• Support patients, families, and referral partners through the referral/admission process.
• Provide hospice information and goals of care conversations with patients and families.
• Facilitate initial patient consent process.
• Attend weekly meetings to report successes and growth opportunities.
• Meet or exceed performance expectations set by leadership.

Position Summary

The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners.

The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process.

• Support patients, families, and referral partners throughout the referral and admission process
• Provide hospice informational / goals of care conversations with patients and families
• Facilitate initial patient consent process
• Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
• Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership
