Job summary

Home Health Care, Inc. is seeking a Hospice Growth Executive in Minnesota to expand our referral network and drive timely access to high-quality end-of-life care.

You will build trusted relationships across hospitals, SNFs, clinics, and physicians to generate referrals and coordinate with intake and clinical teams for seamless handoffs. The role emphasizes educating partners on hospice eligibility and benefits, with a focus on expanding our market coverage in the Twin Cities Metro.

Qualifications

• Bachelor's degree preferred.
• Hospice sales experience preferred or a successful background in medical device, pharmaceutical, DME, home health, or post-acute sales.
• Proven ability to build relationships and grow a territory.
• Self-motivated with strong communication and organizational skills.
• Valid driver's license and willingness to travel throughout the Twin Cities Metro.

Responsibilities

• Develop referral partnerships with hospitals, skilled nursing facilities, assisted living communities, physicians, clinics, and case managers.
• Educate referral partners on hospice eligibility, services, and the benefits of early hospice conversations.
• Generate new referral opportunities through consistent outreach and relationship building.
• Collaborate with intake and clinical teams to ensure a seamless referral experience.
• Monitor referral activity, admissions, and conversion metrics.
• Represent Superior Hospice with professionalism, integrity, and responsiveness.
