Job summary

Optimal Care, a stand-alone hospice provider with home health and physician services, seeks a Hospice Client Support Executive to grow referrals and educate partners.

You will build relationships with physicians, hospitals, senior living communities, SNFs, and other providers to increase access to hospice care, while leveraging your existing connections. Competitive base salary with unlimited incentive potential and comprehensive benefits are offered.

Qualifications

• High School Diploma or GED.
• Minimum 1 year of sales experience.
• Proven interpersonal, coordination, and leadership skills.
• Practical knowledge of hospice and palliative care.
• Willingness to travel daily to local referral sources and partners.

Responsibilities

• Build relationships to expand access to hospice care and promote Optimal Care’s services.
• Educate referral sources and coordinate the referral process.
• Develop partnerships with physicians, hospitals, senior living communities, SNFs, and other providers.
• Promote hospice referrals and ensure patients receive needed support.
• Serve as a liaison between Optimal Care and healthcare partners.
