Job summary

Affinity Hospice is seeking a driven Hospice Consultant to drive referral growth by building trusted relationships with physicians, hospitals, long-term care facilities, and community partners. This full-time, field-based role combines strategic territory management and consultative selling.

Qualifications

• 2+ years of healthcare sales or marketing experience, preferably in hospice or home health
• Proven ability to build relationships and influence decision-makers
• Valid driver’s license and reliable vehicle

Responsibilities

• Build and maintain referral relationships with physicians, hospitals, and long-term care facilities
• Educate referral sources on hospice eligibility and Affinity’s differentiators
• Identify and close referral opportunities by engaging key decision-makers
• Plan and execute territory activities using data and market insights
• Document referral activity and market intelligence
• Participate in community education, speaking events, and outreach
